Emerging CARE
Farm Bureau Health Plans is partnering with UMR to offer additional benefits for members living with specific chronic conditions and behavioral health, including substance use disorder at no cost to you. The following is a brief overview of your enhanced benefits.
Added benefits for achieving better health
Chronic condition support
Our UMR CARE team will assist those members identified as living with chronic conditions to provide education on medication adherence and ongoing management of your illness.
Conditions include:
- Diabetes types 1 and 2
- Coronary artery disease (CAD)
- Chronic obstructive pulmonary disease (COPD)
- Behavioral health, including depression, anxiety, substance use disorder and additional behavioral health conditions
- Congestive heart failure
- Asthma
- Hypertension
Behavioral health and substance use disorder
UMR CARE will provide a proactive approach to assist families and individuals at risk of inpatient or outpatient services for behavioral health, substance use disorder. Our approach includes coordination with providers as well as member education.
The CARE app
The CARE app, powered by Vivify Health, allows us to meet members where they are by connecting them to CARE clinicians, coaches and other resources through their mobile device. CARE nurses can view individual health metrics from self-reported data or synchronized monitoring devices and are able to virtually connect with members by text, email or face-to-face via streaming video.
CARE Cues
Confidential email notifications with personalized reminders of actions individuals can take to improve their health. CARE Cues includes prompts to recommend preventive care, such as annual screenings, mammograms or cervical cancer screening, diabetes monitoring or heart health care.
